Your Cancer Care Team
The Cancer Care Team provides case management support and holistic care for patients who are facing the challenge of cancer. Patients often need additional support and care beyond that provided in the clinical setting. The Cancer Care Team works in partnership with patients and their families to identify and prioritize financial, social, emotional and spiritual needs.

Types of Services We Provide
The Team strives to empower patients to overcome the barriers which may keep them from maximizing their treatment. It helps them navigate the continuum of cancer services.

The team brings together the expertise and skills of a variety of healthcare disciplines:

• Nursing
• Pastoral Care
• Occupational Therapy
• Social Work
• Nutritional Therapy

Who and Where We Serve
Services are targeted toward patients on active treatment for cancer and who live in Central Texas. The Team serves patients in a variety of settings including clinics, physicians’ offices and patients’ homes.

No Charge for the Cancer Care Team
There is no charge to the patient, the family or healthcare professional for services provided by the Cancer Care Team. Team services are funded entirely by a grant form the Shivers Cancer Foundation.
